You have decided to play a major part in your own health. Once you have clearly set this intention, our efforts to realize your potential will be primed.
As your naturopathic physician I can assure you that I will act according to the precepts that I have sworn to uphold and do all that I can to ensure that your experience here is a positive one.

Please print and fill out these forms prior to your first visit in order to better utilize your time at the clinic. If you do not have access to a printer they can be picked up in office, mailed to you, or filled out prior to your visit. Please allow 15 -30 minutes to fill them out.

"It takes time to get better". Some patients have spent many years with chronic medical problems unsolved by conventional medicine. Some are currently receiving positive and necessary treatment from one or more medical doctors or other health-care providers. Some are simply not feeling well and want to improve their general health. Whichever scenario applies to you, it is important to realize that it takes time to feel better when using naturopathic medicine. We usually tell patients to expect to visit us at least four (4) times and to expect to wait approximately two (2) months before noticing significant changes. Some patients of notice changes much sooner, but as a patient, we ask you to be patient! We are confident that with the necessary information and a consistent effort from you, we can help you; but it may take some time.
Your ND is trained in several modalities (treatments) and any or all of them may be employed on a case-by-case basis.
| VISIT DURATION | |
An initial visit with your Naturopathic Physician can take up to 1½ hours. This gives us ample time to get a grasp of your overall health and begin a wholistic individualized treatment plan. Hopefully you will have completed the lengthy intake form and review of systems before we begin our visit. The information on this form is to be seen by your physician only, and will be kept in strict confidentiality. During this visit or the next your ND will perform a hypothesis-driven physical exam and order lab tests. Your subsequent visits may take anywhere from 15 minutes to 60 minutes, depending on the case. The fee schedule for these services is time-based, and is attached.
| PRODUCTS | |
At the clinic we provide high quality supplements and other natural health products. It is important that you are aware that you are free to purchase your supplements at a pharmacy or health food store of your choice. By no means do you have to buy the products that we supply.
The products that we supply are on hand so that your ND can be sure of the quality and availability of the interventions, and not depend on other suppliers to have what you require.
| MISSED APPOINTMENTS | |
If you are not able to make your appointment, please notify reception as soon as possible. If you are able to do so with 24h notice you will avoid the cancellation fee. Cancelling your appointment well in advance should not be a problem, as we will give you a reminder call a few days to a week before your next appointment.
Cancellation fees are 50% price of a 1h visit if same day, and 100% of a 1h visit if no notice is given at all.
